Unless there is certainty about the emission of something from the woman, the default is the persistence of purity, and there is no need to repeat ablution or prayer. The wetness in the underwear is usually normal vaginal discharge, which is pure but invalidates ablution, unless there is an indication that it is madhy (pre-seminal fluid). Ghusl (ritual bath) is not required for the emission of discharge unless it is confirmed to be maniy (seminal fluid), which necessitates ghusl.
