It is not permissible for a Muslim to directly engage in prohibited contracts or to assist in them, as Allah Almighty says: "And cooperate in righteousness and piety, but do not cooperate in sin and aggression." If insurance contracts are commercial, then it is not permissible to fill out their forms, even if ordered by the supervisor at work, because this falls under assisting in sin. However, if the insurance is cooperative and tafaful (mutual), then there is no harm in it.
