What should I do about the involuntary and continuous discharge of semen, and is ghusl (ritual bathing) for major ritual impurity and cleaning the place required every time?

If semen is discharged in a gush with pleasure, followed by a weakening of the body, then ghusl (definition) (ritual bath) becomes obligatory, based on the Prophet's (peace be upon him) saying: "Water is only from water" (i.e., ghusl is only required after seminal discharge). However, if it is discharged without pleasure and not followed by a weakening, then ghusl is not obligatory; rather, only wudu (ablution) is required, based on the Prophet's (peace be upon him) saying to Ali (may Allah be pleased with him): "If you ejaculate, then perform ghusl for janabah (definition) (major ritual impurity); but if you do not ejaculate, then do not perform ghusl." An exception to this is the discharge of semen from a sleeping person, in which case ghusl is obligatory in all circumstances.
