If the wife was taken back after the expiration of her iddah (waiting period) from the first divorce without renewing the marriage contract, then the return is invalid, and the subsequent divorce does not take effect. A new contract must be made to take her back. However, if you took her back with a new contract and then divorced her, the divorce is effective. Divorce does not take effect with doubt in it or in its condition. The last divorce is considered valid and effective even if it occurred while the woman was menstruating. Your writing down the divorce three times and informing her of it, due to your assumption that three divorces occurred, does not result in the occurrence of three divorces as long as the matter is otherwise. Therefore, you have divorced her twice, and you may take her back before the expiration of her iddah. It is advisable to present the matter to a Sharia court or to trustworthy scholars.
