The answer

Purity from menstruation and post-natal bleeding (nifas) is recognized by two signs: the white discharge (al-Qassah al-Bayda’) or complete dryness. Since the questioner did not experience complete dryness, and the yellowish discharge continued, and a small amount of blood appeared two weeks after cessation, she is still in her nifas. Her prayers before achieving purity are incorrect. The maximum period for nifas is forty days. If she achieves purity before that, she should perform ghusl and pray. If the yellowish discharge or blood continues beyond forty days, it is considered istihadah (irregular bleeding) unless it coincides with her regular menstrual period, in which case it is considered menstruation.
