It is permissible for the sister not to wear the hijab and to fast Ramadan and make up for it outside of its prescribed time if doing so would cause her harm. This is based on the Almighty's saying: "Allah does not burden a soul beyond its capacity." Scholars have stipulated that whoever believes in a disbelieving land and is unable to openly practice their religion is not burdened with what they are unable to do.
