The question

Are the religious and moral commitments and personal sacrifices I have made throughout my life sufficient to spare me the torment of the grave and make me one of the people of Paradise, especially with my feeling of exhaustion, desire for death, and intense fear of the torment of the grave and Hellfire?

The answer

Praise Allah for the blessing of steadfastness, and ask Him for more, for constancy, and for help in expressing gratitude. What you are doing in terms of raising and caring for children is a great reward for you. Do not wish for death because of ordinary life difficulties, for blessed is he whom Allah guides to His obedience. The Messenger of Allah (peace and blessings be upon him) forbade wishing for death. The path to salvation from the Fire and happiness in this world and the Hereafter is steadfastness, obeying Allah's commands, and refraining from His prohibitions.
