In which Surah was the Prophet (peace and blessings be upon him) made a Prophet, and in which Surah was he sent as a Messenger?

The Prophet, may Allah bless him and grant him peace, was informed of his prophethood with the first five verses of Surat Al-Alaq, and he was sent with Surat Al-Muddaththir. The period between the revelation of the beginning of Surat Al-Alaq and Surat Al-Muddaththir extended to six months.
