Will a person be held accountable for internal thoughts if they are deliberate and stem from an ill omen, and what is meant by "acting accordingly"?

"The whispering of the self (hadith al-nafs) regarding a sin is forgiven if it is merely a fleeting thought and one does not act upon it. However, commencing the sin is recorded, and likewise, the firm resolve to commit it is recorded as a bad deed. Persistence and firm resolve (to sin) are a disobedience recorded as a bad deed. If one then commits the sin, it is recorded as a second disobedience. If one abandons it out of fear of Allah, it is recorded as a good deed. Fleeting thoughts that one does not dwell upon are not recorded. The accountability for a firm, settled intention of the heart is indicated by the texts of Islamic law."
