Is a divorce effected by the statement, "By Allah, if you do such and such, you are divorced," if the husband's intention was to threaten and intimidate her from doing the act, and he was serious about the divorce occurring if she did it, and the wife has done that act multiple times?

The divorce occurred due to the wife's actions, and her husband suspended her divorce on it. The breach of an oath is not repeated by the repetition of the action, because the oath is dissolved by the first breach. If this was not the third divorce, reconciliation is permissible as long as the wife is within her waiting period (iddah (definition)). Both spouses should be keen to preserve the marriage and avoid resolving problems through divorce.
