Is a man's addressing his wife as "my mother" or "O my mother" considered Dhihar, necessitating its legal rulings, as in the Almighty's saying: "Those among you who declare their wives to be like their mothers – they are not their mothers"?

This phrase is not ẓihār unless the husband intended it as such. It is better for a husband to avoid calling his wife "my mother" or "my sister," because it is disliked for a man to address his wife by the name of someone unlawful for him to marry, and because the Prophet, peace and blessings be upon him, forbade it.
