What is the ruling on burying women with men, and what if a woman has already been buried with a man?

It is not permissible for a man and a woman to be buried together in one grave unless there is a necessity, and with a barrier placed between them. If this occurs without necessity, then it is legislated to exhume the grave to bury each of them separately, unless there is a fear of the deceased's decomposition.
