The answer

If it is merely a fleeting thought (khatirah), it does no harm. But if it settles, it is a form of suspicion (dhann), for which one is sinful if it concerns someone whose outward appearance is righteous, unlike someone whose outward appearance is evil, in which case one is not sinful.
